Wyoming Access Showing Signs of Success and Growth in its First Year
Wyoming Access providing community-based accessible care seeing positive results.
- Denver, CO (1888PressRelease) November 13, 2013 - In the Spring of 2013, Colorado Access partnered with the Wyoming Department of Health to establish Wyoming Access, a unique program that provides care to youth with Medicaid and who have complex behavioral and emotional health needs.
Wyoming Access provides comprehensive, individualized, and cost-effective care coordination. It serves youth and families living in the southeastern communities of Wyoming, including Albany, Carbon, Converse, Laramie, Goshen, Niobrara, and Platte counties.
Wyoming Access leverages the High Fidelity Wraparound model to focus on strength-based service planning, coordination across providers, intensive care management, and home and community-based alternatives to costly residential care.
The primary goal is to develop a single plan of care that empowers families and increases resiliency. "We are providing the kind of intensive and comprehensive care that will keep youth with their families and in their communities, with everyone stronger as a result" said Shelby Kiernan, Program Director of Wyoming Access.
Wyoming Access has put emphasis on community outreach, and they've seen positive results. Youth are being referred to Wyoming Access through providers, educators, youth serving agencies, and community and advocacy groups. Shelby Kiernan adds: "This program allows us to share resources and programs in rural settings. We're currently working with a small group of youth and look forward to healthy outcomes with them. We have staff working locally in the community to ensure we meet the needs of all our members."
Through positive, empowering results for the families and communities they work with, Wyoming Access expects to expand beyond its current focus and build on their partnership with the Wyoming Department of Health.
