Services include routine health checks and physician consultations, medication management and wellness programs. Each is administered by a member of the community's fully licensed multi-disciplinary team of registered nurses, certified home health aides, personal care aides and homemakers.

Following this evaluation, a physician and case managers develop a customized care plan in consultation with the patient, family members and his or her personal physicians. Results are monitored daily and communicated to each patient's personal physician.
