It solves the consumer’s problems of long wait times at the registration counters and physician rooms. This waiting in crowded spaces takes a toll on the elderly and sick. Consumers are looking for anywhere anytime 24x7 care like in E-Commerce transactions. ActiveCONSULT supports this consumer vision and gives the ability for a similar convenience in healthcare delivery.
It is now accepted that a Hybrid care is the way forward that gives the best of both worlds to patients. A judicious mix of clinic and virtual visits serve the best interest of patients. Here minor ailments can be addressed through E-Consults interspersed with comprehensive reviews in the physician’s office/clinic. . Patients are served by Virtual visits for activities such as sharing test results, prescription refills, or routine follow-up’s etc.
ActiveCONSULT is a clinically tested and proven method of triaging a patient for virtual consult and has a comprehensive workflow that starts with determining the type of visit namely Preventive Consult, Second Opinion, Sick Event or Specialty consult. A common work flow triages a patient for a clinic visit of a virtual visit based upon the condition and severity.
ActiveCONSULT enables distributed care teams at different locations to cooperate to deliver care to patients in extended areas such as tier 2 & 3 cities and rural areas. The distributed Care teams can interact between themselves in the patient’s interest virtually to provide specialist care to patients remotely. This extends the reach of specialists to enabling quality care that can be executed by primary care physicians.
ActiveCONSULT Workflow: It starts with asking a patient on whether it is an existing problem or a new problem and then after looking at the onset and type & nature of symptoms suggest going to the emergency room or the clinic or the case is fit for a virtual consult. Having filled the details the patients encounter is created and the patient can selects a physician within his care network or on the Active Wellness network. It then takes Legal consent for the consult and sends the request to the doctor. The doctor accepts the request enabling the patient prepares for consult by uploading details along with audio and video files for his problem.
At the appointed time when he clicks on the Link he is taken to a waiting room where he waits till the doctor ushers him in. The doctor completes the consult prepares the encounter notes and writes the prescription & / or tests and closed the encounter. This is further facilitated with cancellation and rescheduling options for both the patient and the treating team.
The personal health records store the transaction and become a part of the patient history for reference in the next visit if needed. This prepares a continuity of care record that is available for reference later.
ActiveCONSULT also enables physician’s to conduct bedside visits to patient in home care and cannot travel. This also enables starting first aid to patients at home in case of sick events that have patients waiting for the ambulance to come and stabilize the patient in the window that enables recovery fully without permanent damage.
ActiveCONSULT is a comprehensive end to end hybrid care model that gives end to end quality care delivery that is based upon scientific principles. It fills the gap in care that exists in the community for extending the reach of evidence based medicine to the consumers who are currently dependent on community members for care advice.
